Services included in membership

The plan offers members a comprehensive package of dental benefits. The plan covers diagnostic, preventive, restorative, and some cosmetic services.  exclusion – invisalign, porcelain veneers implant and implant related services.

2 periodic exams per member per enrollment period

2 Xray series (bitewings) per member per enrollment period

Prophylaxis only, not advanced hygiene services, such as scaling and root planning or full mouth debridement.

Covered Services

Preventive & diagnostic – exams, Xrays, sealants, prophy (regular cleaning)

Basic Restorative – periodontal*, endodontic*, oral surgery composite fillings (tooth color)

Major – Crown & Bridge, Denture and Partial

*Treatment done by specialists outside of our office in orthodontics, periodontics, periodontics or oral surgery is not covered
